The Meadow Park Rehabilitation & Healthcare Center staff is available to answer your questions, meet with you, and accept referrals at any time. Meadow Park offers a range of specialized services designed to meet your patients’ medical, rehabilitation and social needs. For your convenience:
- Our Outreach Staff Members are available to see and evaluate patients for placement at any time
- Our Admissions Office is staffed Monday through Friday from 9:00 AM to 5:00 PM
- Our Admissions Coordinator is on call weekends to accommodate weekend admissions
- We are committed to responding rapidly to PRIs and are always accessible to discuss placement with you and/or your patients and family members
- Medicare, Medicaid and most private insurance is accepted
- We are able to accept residents who speak all languages. Our staff is multilingual and uses communication boards and translation services
- Meadow Park is equipped with the latest medical technology and rehabilitation equipment to provide patients with the highest standard of care
